Effective January 1, 2011 If applicable, this form must be completed by a certified testing agency and submitted to the Building Department for all residential structures. Occupancy of the structure cannot be allowed if this requirement has not been completed.

Effective January 1, 2011 This form must be completed and posted within 3 feet of the electrical supply panel for all new residences. Certificate must be completely filled out and posted at time of final inspection.
