We are providing you with copies of the forms that the Department of Revenue suggests you use when you certify your levy to the Board of County Commissioners Department of Revenue Levy Certification Form.
In addition, the Department of Revenue suggests that you use this form for your Resolutions/Ordinances Department of Revenue Ordinance and Resolution Form.
